Registered Nurse-Health Services Coordinator

Ada S. McKinley Chicago, IL (Onsite) Full-Time

Salary: $78,540.00

Schedule: Monday - Friday with some flexibilities,

Weekend On-Call Rotation: Every 3 to 4 weeks

Local Travel: 50%

Summary: The RN Health Services Coordinator is a position for a registered nurse with a current Illinois RN license to provide professional support for community living arrangements for individuals with developmental disabilities. Will assess, evaluate, and coordinate medical, health, and safety needs of persons served by coordinating and overseeing Direct Support staff on medical needs and medication administration.

Supervises (Position Title): None

Principal Duties/Responsibilities:

  • Provides health evaluations to persons supported in the residential programs.
  • Monitors and updates physical examinations, vision and hearing examinations, immunization records, and medical treatment plans.
  • Monitors clients' health records to ensure that files are current and appropriate follow-up is being implemented.
  • Provides medication monitoring to individuals receiving services. This includes completing initial and annual updates of the Nursing Services Packet.
  • Provides training to designated staff in medication administration.
  • Provides consultation to staff on healthcare-related issues, medications, health screening, and any other health-related matters.
  • Relates clients’ health concerns with the PCP, the lab, the pharmacy, and all other health service providers for routine and specialty services pharmacy.
  • Participates in the admission and discharge process for individuals in the community living arrangement program.
  • Follow up with persons served in hospitals and Rehab to assess their status before returning home.
  • Participates in interdisciplinary meetings for annual staffing and other care-related meetings.

  • This position requires the individual possess the ability to coordinate effectively with other team members, other Agency resources, and outside services in addressing client needs.
  • This position requires the individual possess an understanding of evidence-informed practices, engagement strategies, boundary setting, concurrent documentation, working in a community setting, organizing, motivational interviewing, and family systems.

Mental/Physical Demands: Ability to solve practical problems and deal with various concrete variables in situations with limited standardization. Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients or employees of the organization. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand, walk, and sit. The employee is occasionally required to use hands to finger, handle, or feel, and reach with hands and arms. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Environmental Conditions: The noise level in the work environment is usually moderate. Work is performed in a residential or office environment.

  • This position requires a minimum of an associate’s degree, bachelor’s preferred valid license as a Registered Nurse in the State of Illinois
  • Requires two years of successful direct nursing with Developmentally Disabled adults.
  • Preferred that the individual have the requisite training and experience to qualify as a Certified Developmental Disability Registered Nurse (CDDN).
  • This position requires the use of a personal working vehicle, valid driver’s license and a motor vehicle report acceptable to the agency’s broker as well as having proof of current vehicle insurance.
  • Must successfully complete criminal background check as determined by the State of Illinois Department of Human Services.
  • Must clear a registry verification with the Illinois Department of Public Health.
  • Preferred Illinois RN Trainer (RNT) certification. Completion of RNT is required within 12 months of hiring.

Additional Qualifications:

  • Demonstrates the experiences, beliefs, attitude and awareness that indicates cultural sensitivity to the persons supported.
  • Ability to partner with persons supported individually, in family, to help them mobilize resources and their inner capacity to address problems they face.
  • Ability to coordinate effectively with other team members, other Agency resources and outside services in addressing persons supported needs.
  • This position requires the use of a personal auto, a valid driver’s license and a motor vehicle report acceptable to the Agency’s broker as well as having proof of current vehicle insurance. Must successfully complete criminal background check as determined by the State of Illinois Department of Human Services. Must clear a registry verification with the Illinois Department of Public Health.
