Community-Based Clinician (CBC-MHP)

Ada S. McKinley Chicago, IL (Onsite) Full-Time

Basic Function: Under the Manager of Behavioral Health Services, provides community-based clinical case management, individual, family, and group counseling, service linkage, and therapeutic support services to children, adolescents, adults, and families who are experiencing mental health and/or substance abuse disorders. Services can be provided in various settings, primarily in the home or community.

Supervises (Position Title): None

Principal Duties/Responsibilities:

  • Interviews or conducts initial behavioral health service screenings, secures background information, and provides information and referral services to determine client needs and appropriateness of agency/program services.
  • Under the clinical direction of a Qualified Mental Health Professional (QMHP), helps develop a comprehensive behavioral health treatment plan for clients receiving behavioral health services.
  • Assures that all services identified and authorized are provided, monitored, and documented in accord with agency standards of care.
  • Assures that all clients are served within agency timeframes and in coordination with schools, hospitals, primary healthcare providers, and other agencies involved in the child/family’s life.
  • Assures that all required and identified releases and exchanges of information with other providers of services are obtained and included in the service plan in accordance with standards of practice and the rules of confidentiality.
  • Establishes formal and informal working relationships with educational, medical, social, recreational, family support and concrete services to meet the needs of clients.
  • Documents service provision in case-notes in accord with agency standards, provides and documents services in accord with the productivity and documentation standards of the agency.
  • Participates in the development and presentation of treatment planning reviews at least every six months of service.
  • Provides all services in accord with agency, accreditation, and Medicaid standards.
  • Maintains current knowledge of client resources and agency procedures, including current knowledge of mental health diagnosis, reporting requirements of the Illinois Departments of Children & Family Services, and Department of Human Services, Offices of Mental Health & Developmental Disabilities.
  • Understands and accepts the responsibilities as a mandated reporter of child abuse & neglect as described in the Abuse and Neglect Reporting Act as well as confidentiality rules and regulations set forth by the agency, Offices of Mental Health, Department of Children and Family Services, and Office of Alcoholism and Substance Abuse as applicable.
  • Provides for and transports clients in personal or agency vehicles to therapeutic activities included in the treatment plan.
  • May perform after-hours intervention upon direction of a QMHP.
  • Performs other duties commensurate with the responsibilities outlined herein.

Requirements:

  • This position requires a minimum of a bachelor’s degree in a behavioral health discipline including social work, psychology, or counseling
  • 3 years of experience and/or for the candidate to be in the process of completing a master’s degree in the behavioral health disciplines mentioned. A licensed practical nurse license pursuant to the Illinois Nursing Act of 1987 would also be considered.
  • The position requires the use of a personal auto, valid driver’s license, and Motor Vehicle Report acceptable to the agency’s broker and verification of insurance coverage.

Mental/Physical Demands: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Work requires prolonged concentration using a computer in analyzing and manipulating data. While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel and talk or hear. The employee frequently is required to sit. The employee is occasionally required to stand and walk. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Environmental Conditions: The noise level in the work environment is usually moderate. Duties are performed in a typical office environment. No unusual or adverse working conditions exist.
