Basic Function: To provide support to the case work staff.
Principal Duties/Responsibilities:
- Transports clients to various appointments, i.e. doctor appointment, sibling visits, prison visits, therapeutic appointments, school transportation, etc.
- Monitors visits and makes notes of observations
- Documents observations as case notes in Word and forwards to case worker
- Assist Program Supervisors and Case Managers in planning and implementing client activities
- Pick up important documents for client files, i.e. birth certificates, death certificates, etc.
- Provides outreach via telephone to foster and biological parents regarding scheduled visits
- Other duties as assigned
- Mental/Physical Demands: Ability to use common sense, patience and understanding to carry out instructions furnished in written, oral and diagram form. Work may require prolonged concentration using a computer in analyzing and manipulating data or designing spreadsheets. While performing the duties of this job, the employee is regularly required to use hands to finger, handle or feel and talk or hear, sit, stand and walk. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The employee will occasionally lift up to 10 pounds.
- Environmental Conditions: The noise level in the work environment is usually moderate. Duties are performed in a typical office environment. No unusual or adverse working conditions exist. Employee is expected to follow all safety rules and regulations.
